How to Join the Project as a Participant

How to Participate? Becoming one of the 25 fortunate participants on this trip can be done in 7 easy steps. Please review the Forms and Payments to be Completed and submit all forms and deposits in before the seats are all gone. All trip related information submitted below will be kept confidential and viewed only by the ADVENTravel adult leaders. Step 1. FBA ADVENTravel Project Reservation Form in order to temporarily save a seat on the trip. The participant will have 7 days to review the trip information and decide whether to complete steps 2 through 7 and parmenantly hold a seat.
Step 2. FBA ADVENTravel Enrollment Form This form includes the newly required motivation statement. Forms for steps 3, 4, 5 and 6 must be downloaded, completed, then mailed to FBA. (A parent or legal guardian must read carefully and sign forms 3, 4, and 5 for participants younger than 18 at time of the form’s completion. Spanish version of these forms are available upon request).
Step 3a. The project deposit can be paid online via PayPal by clicking here; or
Step 3b. The project deposit can be paid by a check made payable to Forever Bloom Alliance and send via regular mail along with forms 3, 4, 5, and 6. The project deposit is $100.00. The $100.00 deposit and subsequent payments are refundable up to May 1, 2016. Visit our Payments page to view the payments schedule.  After May 1st the $100.00 deposit is non-refundable.
Step 4. Download and complete the Participant Registration Form.
Step 5. Parent Permission Form (For participants under age 18)
Step 6. Health Statement Form (Participants Age 18+) or Health Statement Form (Participants Age 15 – 17) with copy of insurance card attached
Step 7. Participant Rules and Guidelines (Participants Age 15 – 17)
